The licensed use of a multiple-dose pen is four doses of 0.6 mL. Part-doses, or drawing from the cartridge with a syringe, are outside the product information, and accuracy is not guaranteed at the end of a cartridge.

? ‘Golden dose’ is what people call the solution still in the cartridge after the counter has delivered its four labelled 0.6 mL doses. Manufacturing overfill exists so that four full doses can always be given; it is not extra medicine the product is licensed to deliver, and the amount is not specified on the label.

Turning this on makes the “doses per pen” figure count that leftover volume. It changes nothing about the dose itself. We neither recommend nor discourage it — but note that dose accuracy is not guaranteed at the end of a cartridge, and this is outside the product information.

How the number is worked out

One constant does all of it: 1 click = 0.01 mL. A labelled dose is 0.6 mL — 60 clicks — and the pen strength is the milligrams in it.

clicks = target mg ÷ pen mg × 60  and  mL = clicks ÷ 100

5 mg from a 12.5 mg pen: 5 ÷ 12.5 × 60 = 24 clicks, or 0.24 mL. Checking it the other way: 12.5 mg in 0.6 mL is 20.83 mg per mL, and 0.24 × 20.83 = 5.0 mg.

0.01 mL is also one unit on a U-100 insulin syringe, so the number is the same whether you count clicks or draw it up.

The pen styles. Both give the same four 0.6 mL doses; only the leftover differs. Older cartridges hold about 3.2 mL and newer ones about 2.6 mL, against 2.4 mL of labelled doses.

Small print

Mounjaro (tirzepatide) is a prescription-only medicine. Nothing here is a recommendation to split doses, to extract extra volume, or to use any device other than as its product information describes.
